If I make chicken soup and I include carrots and onions to make the stock, do I need to count the carbs in these if I don't eat them and discard them afterwards?

Well, depending on how long you leave them cooking in the stock, I imagine they might impart some carbs to it, since long cooking will break down the cellulose of the cell walls. So you might want to count them at 25-50% of their value. This is usually negligible when divided up by servings.
