I don't know the glycemic index number on liver, but I suspect it can't be too bad, considering it's glycogen (glycerine). Glycogen isn't one of the worst carb offenders, is it?
I would say eat it and see how things work for you. I have never worried about it, personally. Liver is one of the richest B-vitamin sources you can possibly eat, especially B-12.
